Model release form...

I decided to write my own model release form as i couldn't find one that said what i needed it to say. The only reasoning behind me taking a model release form is to have their permission in written form to use the images i take on that day during and after my studies in university. I have also left an option for them to leave their email so that i can send them the finished images afterwards.

I hereby give permission to Amelia Johnson to use the images taken today in all forms and media during and after her studies at The University of Salford.
